550 not indexing

I am having issues with crushing and disfiguring primers when "trying" to load 223 on my 550. I am using LC '11 that I am swaging with my Super Swager and I have also tried some non military brass. I thought I was not swaging the LC enough so I tried swaging more to the point where the case wouldn't fit in the swager. I was still crushing or disfiguring primers. Well tonight I noticed that my plate is not indexing centered as per this picture. I have tried lossening the plate and tightening the plate to try to get it to center with the primer feed. Is there a possibility that I could have a conversion kit/plate that is off?
