More information would be helpful for diagnosing this, such as caliber, photo of flaring, etc. But it sounds like the failsafe rod is not set up correctly. Be sure the bent end goes into the oval hole in the rear plate, and comes out the round hole in the front plate. Next, push the handle forward as if seating a primer, and hold it forward. While holding the handle forward, tighten the blue wing nut until the coil spring above it is compressed by at least 1/3 of its length. This ensures the powder bar is fully retracted to pick up the next powder charge.
