Ejector Wire Jumps Out of Position

Not on every round, but occasionally, the ejector wire will push over to the groove on the other side of the shellplate bolt. I've checked the adjustment of the shellplate, removed it, and replaced it. As far as I can tell, the ejector wire is being pushed out when a round passes from Station 4 to the ejection chute, and is caught between the ejector wire and the locator button. I've looked to see if the diameter of the case is causing this, as it's possible that different manufacturers have slightly different diameters on the base of their cases. I've not found a particular brand of case to be the cause.
I've never encountered this with the .38 Special, .357 Magnum, or .45 ACP that I've loaded. I thought maybe the shellplate was inserted incorrectly into the press, but I've reseated it, ensuring that it is in alignment with the detent ball and detent lever. The shellplate advances correctly, and everything seems to function properly, except for the ejector wire jumping over to the other side of the shellplate bolt.
I've tightened the bolt down as far as it will go, and then backed it off about 1/4 turn. The bolt has been lubricated, as well. I don't know what's causing the ejector wire to jump out of the shellplate groove, though. It's a minor issue, as it snaps back into place rather easily, but I don't believe it should be doing that at all.
Any suggestions?

It appears that the case gets pinched between the locator button on station 4 and the ejector wire, and that pushes the free end of the ejector wire over to the other side of the shellplate bolt groove. It doesn't happen with every case, and I can't pinpoint a particular case that's causing it, but the locator button doesn't spin freely in its slot when this happens.

It seems that you have the correct #5 Shell plate and #3 buttons, This happens to me when i do not properly seat a primer. During bullet production it is possible that you are not fully seating the primers and they get hung up on the shell plate and instead of falling out they push against the ejector wire and it defaults out of place to indicate that something is not correct. I would check to make sure that you are properly seating the 9mm primers and that they are at least flush with the bottom of the shell.
