Powder Measure Inconsistant Drops/Actuation

Any ideas or pointers on what I might be doing wrong here?  This is a powder drop of 5.2g of BE86 on  a 115g 9mm load using already primed mixed 9mm brass from American Reloading.  I'm using a 550b with the Dillion powder measure.  Sometimes the powder measure does not fully actuate and no powder drops despite the case being fully inserted and the roller lever fully down....and I have to keep my eye on it and push it flush with my finger to drop the powder.

The powder measure die is not threaded deep enough into the toolhead. Use a sized case to activate the measure, and adjust the die down until the case mouth is flared out at least .010" larger than a sized, unflared case mouth measures. When properly adjusted, the powder bar should go full travel before the handle bottoms out.
