powder measure problems

I have an old style powder measure the one that uses two springs to help return the powder slide bar. I was using it the other day and noticed the powder bar kept slamming back. I found that the bellcrank cube had come out of the powder bar and was laying on the floor. I could find nothing wrong so reinstalled it making sure everything was tight. I managed to load a few rounds and this time saw it fly out again. I checked and found it was cracked. I replaced it with another brand new one out of another powder measure. I loaded two rounds before the new came out. Any ideas on what is happening here. Worn out bellcrank? BTW I switched to another powder measure and it works fine same new bellcrank cube installed in it no problems at 100 rounds.

Check the bolt that secures the bellcrank to the powder measure body. It should have a bowed washer between the head of the bolt and the bellcrank. Also, remove the bellcrank, check the powder measure body for tiny cracks where the bellcrank bolt threads into it.
