Reloading 357 and 38 special

I am having squib loads with 357 and 38 specials with Unique powder. I visually inspect each load at third stage ensuring there is powder in the cartridge.
I have reloaded thousands of rounds of 45ACP, 9mm, 223s.
Suggestions?

Do you tumble your cases before or after depriming? Are you wet or dry tumbling cases?

Do you use the same powder, &  measure for all calibers ?  If not try the powder used in 45 acp, for the 38 spec.  I use Bullseye, for 380, 38 spec, 357, & 45 acp.  It may be a metering problem.  I reload in stages, sometimes over several nights.  The last stage, is to add powder, & then the bullet, no stacking them in a tray.  The newly completed round, goes right into a box.   I haven't reloaded thousands of rounds, but enough to keep my guns happy.  Even doing it in stages, I have still had a squib.  Shooting a brand new S&W, 380, Body guard.  Had a different sound, checked, & the bullet was sticking out of the barrel.  Good thing I wasn't doing quick double taps.  Thanked the "Boss", for saving my can. 

That happened to me twice.  The cause was that i had glops of media too wet because I failed to let the tumbler spread it out or mix it before I added cases. 
Every so often I would smell something burning, turned out to be the corn cob media.
