Sideways primers

I recently switched calibers from 9mm to 38 and I've been getting sideways primers.

I cycled the press empty for a number of turns and it seems that on the return (down) stroke, as the index plate comes down to the top of the primer cup there's some kind of interference that causes the primer to jump. Basically I think the index plate is just catching the edge of the primer cup.

Do I need to adjust the set screw that's exposed on the rear of the ram when it's at the top stroke?

   Had a similar issue a few months ago. The center screw on the rotating  index table was loose causing a ever so slight misalignment.

     Who would have thought that after 10,000 rounds a little pm was in order.
