Suare Deal Primer Seating Problem

I'm having a problem with the primer system.  As the shell plate turns the brass into the primer station the spring loaded primer cup catches on the bottom of the shellplate die.  When acutating the press with no brass in the press, as the primer cup clears the shellplate the primer is ejected out of the seat and bounces it into the air.  When reloading, in some cases the primer seats properly, other times it flips the primer onto it's side, or up-side-down and is crushed or seated up-side-down.

I have read the other posts on this topic, and have removed and cleaned the primer assembly, and have mesured the slide to top of the primer cup dimension (it is with the published spec.) with no improvement. 

As I watch the shellplate turn it appears that the shellplate may not be rotating quite far enough, and the visible tip of the Index Lever which rotates the shellplate seems to have significant wear, and a gouge in the tip.  Is there a way to adjust the shell plate indexing?  Do I need to replace the Index Lever Assembly? I haven't found anything on this topic so far.  Thanks in advance for the assistance.

First, remove the primer feed from the machine. Then try cycling the handle with a little downward pressure from your thumb. See if the index lever pushes the shellplate far enough around for the detent ball to locate it. If it does not, look on the right side of the shaft towards the back for a set screw#13789. This set screw stops the index lever from travelling too far. Back it out 1/2 turn, then test it.
